Appointments
Please call the office at (717) 846-7846 to make an appointment in advance.

If you're unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.

Your First Visit
Your initial visit will include a comprehensive diagnostic evaluation, which will be recorded in detail in your patient record.  A copy of this will be sent to your referring physician. Information regarding your evaluation will not be released by our office to any one other than your referring physician without your written permission. Please remember to bring your insurance card to your appointment.

Please take a few minutes to download, print out, and complete our Patient Information Sheet and Health History Form. Please bring these completed forms with you when you arrive for your visit. Adobe Acrobat Reader is required to download these forms. The Reader can be downloaded free at www.adobe.com.

Insurance
As a courtesy to our patients, we participate with most insurance plans. Depending on your plan, you may be responsible for a co-pay and/or deductible at the time of service. Our staff is extremely knowledgeable about most insurance plans carried in the York area and will be happy to provide you with more detailed information regarding your particular insurance plan. Below are listed some of the major insurance plans in which we participate. However we also participate in numerous other PPO and HMO plans. Please feel free to call our office and inquire about your insurance plan. ALLIANCE, AETNA, BLUE CROSS, BLUE SHIELD, CIGNA, DEVON, FIDELITY, HEALTH AMERICA/ASSURANCE, HEALTH GUARD, INTERGROUP, KEYSTONE, MAMSI, MEDICARE, PHCS, OPTIMUM, TRICARE, UNITED HEALTH CARE.

All requests for prescription medication and refills must be made during business hours.
